POSITION SUMMARY
The Community Wellness Department (CWD) is a multi-site program operating in San Francisco and Oakland that provides culturally sensitive prevention, early intervention, and supportive social service programming. CWD community programs include social service programming for youth and adults, cultural programming, health education, system navigation/ coordination services, and HIV/AIDS prevention and care. Services are rooted in a holistic provision model that integrates prevention, community engagement, cultural competence and provides linkages to other treatment services.
The Community Wellness Department is seeking a Senior Program Manager to collaborate with the CWD Director to manage day-to-day operations of the department program areas, monitor grant performance, assist with budget management, lead program planning, guide QMIP and other Quality Improvement initiatives, operationalize reporting and evaluation, vision and guide the expansion of departmental services. The Senior Program Manager D&E will specifically supervise the Data and Evaluation team. The Community Wellness Department provides multi-site services in 2 Bay Area Counties at 2 locations (1 SF and 1 in Oakland) to urban American Indians, Alaska Natives, other Indigenous people, and diverse members of the surrounding communities.
The Native American Health Center is an accredited institution and adheres to the standards of excellence set forth by the Accreditation Association of Ambulatory Health Care (AAAHC) and the Commission of Dental Accreditation (CODA).
DUTIES AND RESPONSIBILITIES
Manage and oversee CWD program areas including day-to-day program operations, project performance management, hiring, human resources management and ensuring effective spending2. Conducts audits of database and relevant processes in order to identify problems and perform necessary corrections.
3. Provide direct supervision to CWD Data and Evaluation Team. Lead an integrated structure for all program managers, evaluators, and data assistants to collect, manage, analyze, and prepare data reports to contract and funding organizations, and ongoing QMIP and other Quality Improvement initiatives.
4. Resolve staffing issues and facilitate conflict resolution for staff under direct supervision.
Hire, discipline, evaluate and terminate staff in coordination with the Human Resources department.
5. Function as an integral member of the department’s leadership team, working closely with the Director of Community Wellness Department, Director of Behavioral Health, and Director of Research & Evaluation
6. Support the execution and implementation of initiatives across the department and multiples sites, coordinating with site managers, and analyze data to ensure compliance with accrediting bodies and funding sources.
7. Support departmental strategic planning initiatives and engage in department development, performance management, and team building.
8. Ensures patient and program data is accurately and completely recorded in the agency’s electronic record health system according to contractual timelines and requirements
9. Provide leadership on program development and expansion of program activities to support grant writing. Duties include writing and collecting supporting documents and directing grant communications with funders.
10. Coordinate activities across departments, sites and with collaborating agencies to ensure that program areas are meeting goals and objectives.
11. Document processes, procedures, workflows, and meeting agendas/minutes. Conduct presentations on CWD and NAHC programs at the local, state, and national level and ensure dissemination of information to NAHC staff and community, as appropriate.
12. Member Care: Demonstrate understanding and apply working knowledge of safety policies and ensuring safe member practices.
13. Quality Improvement: Actively participate in internal quality improvement teams and work with members proactively to drive quality improvement initiatives in accordance with the mission and strategic goals of the organization, federal and state laws and regulations, and accreditation standards, when assigned.

16. Must maintain compliance with ergonomic safety standards; be mindful of posture and regularly practice ergonomic stretches.

18. Safety: Take appropriate corrective actions to address matters pertaining to employee health and safety that have been brought to their attention.
19. HIPAA: Keep all protected health information (PHI) confidential and abide by HIPAA guidelines for the release and disclosure of any PHI. Will report unauthorized use of disclosure of PHI immediately, to Site Director and HIPAA security officer.
20. HIPAA: Ensure the protection of all PHI and compliance of HIPAA guidelines amongst staff under your supervision. Take appropriate steps to address HIPAA concerns with staff as necessary.
