Senior Social Worker (Contract Residential Services Coordinator/Liaison)
Summary Incumbent serves as a Senior Social Worker at the GS-12 level located in the Healthcare for Homeless Veterans (HCHV) Program, in the Homeless Program, within the Mental Health Service. Incumbent is assigned as a liaison and Contracting Officer's Representative (COR) in support of VA transitional housing programs Responsibilities Candidates will be reviewed for minimum qualifications as outlined in the qualifications section of this announcement and referred to the Loma Linda VA Health Care System Social Worker Interview panel no later than 60 DAYS OF THE VACANCY CLOSURE. Selected candidates should plan to report to Loma Linda VA Health Care System for fingerprinting and a physical evaluation. VA Careers - Social Work: https://youtube.com/embed/enRhz\_ua\_UU As the Contract Residential Coordinator, the incumbent is responsible for clinical and administrative coordination and oversight of services provided to Veterans enrolled in HCHV Contract Residential Services programs, as described in the HCHV handbook, regulations, and directives. Liaison duties include verification of Veteran status and eligibility for participation in HCHV Contracted Residential Services programs, maintaining admission and discharge dates of participants for billing purposes, reviewing and approving/disapproving readmissions to and extensions of care, and collecting and submitting participant information as outlined by NEPEC evaluations procedures. The senior social worker must demonstrate a high level of professional judgment, which includes the ability to independently conduct complete assessments and make clinical decisions and recommendations. Duties will include oversight of care provided by contractor, provision of technical guidance to contractor, and linkage of Veterans to services and resources as needed to enhance care provided by contractor. Incumbent will appropriately address critical incident reports through follow-up with contractor and any involved Veterans, reporting through the chain of command, and corrective actions as needed. The incumbent monitors the clinical care of program participants and may utilize various treatment modalities of individual and group work, case management, casework, community organization and community outreach activities to address problems that interfere with the program participants' ability to achieve independence. The incumbent develops policies and procedures for program operation and prepares reports and statistics for facility, VISN, and national use. Administrative duties include tracking service activities, including utilization and patient satisfaction, development and evaluation of program guidelines, and monitoring National/VISN quality performance measures. The incumbent may identify best practice programs for consideration of implementation and may support research to identify local veteran needs. Incumbent actively participates in the Homeless Program and Contract Residential Program conference calls and attends training offered by the National Homeless Programs Office and National Grant & Per Diem Office. Incumbent complies with criminal conflict of interest laws and Executive Branch Standards of conduct so as to avoid conflicts of interest while serving as the Homeless Contract Residential Program Liaison/Case Manager. Incumbent works with VA Medical Center multi-disciplinary teams and community agencies to develop more effective and efficient ways of delivering healthcare services to HCHV Contract Residential Services program participants. Incumbent engages in coordination with community agencies, organizations, and groups to learn of services or resources available for homeless Veterans as well as provides current information on services and benefits available for homeless Veterans. Among additional duties, the incumbent may assist the Social Work Section with supporting the national Veteran Transportation Service Initiative. Incumbent may provide clinical supervision for social work licensure or certification, and may service as the field instructors for social work interns. Perform other duties as assigned. Grade Determinations: Social Worker (Program Coordinator), GS-12 Experience and Education: Applicants must have at least one (1) year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Demonstrated Knowledge, Skills, and Abilities: In addition to the experience above, candidates must demonstrate the following KSAs: (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. (c) Ability to supervise multidisciplinary staff assigned to the program. (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). (e) Ability to provide training, orientation, and guidance within clinical practice. Assignments: For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety and be performed by the incumbent at least 25% of the time. Program coordinators at the GS-12 grade level are administratively responsible for a clinical program providing treatment to Veterans in a major specialty area, such as but not limited to: Spinal Cord Injury, Homeless Continuum Veteran Program, Hospice and Palliative Care Program, Suicide Prevention Program, Veterans Justice Outreach, Caregiver Support Program and Community Nursing Home Program. The program coordinator may be the sole practitioner in this specialty at the facility and typically provide direct patient care services in the program area. The program coordinator oversees the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. They may be responsible for the program's budget, developing and monitoring staff compliance with practice, standards and guidelines on documentation, workload, data entry, ethical practice and service delivery. The program coordinator provides analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to enhance the quality of service. They may be responsible for, or contribute to, the program's resource and fiscal management, monitoring control points developing the annual budget, operating within that budget, and accounting for appropriated funds. The program coordinator is administratively responsible for the clinical programming and prepares reports and statistics for facility, VISN, and national use. They provide leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned staff. They initiate and conduct a variety of program or service audits and complete designated clinical practice audits and reports, including productivity assessments. They oversee program operations and evaluations, identifying areas for improvement, gathering relevant data, assessing the data, developing and implementing ideas for improvement and evaluating efficacy of improvement efforts. Physical Requirements: Traveling throughout the medical center and the community is required, as is performing activities involving sitting, walking, and prolonged standing, bending and carrying such items as books, papers, and files. In carrying out responsibilities it may be necessary for the incumbent to travel into the community where he/she conducts interviews with the Veterans, their families, representatives of community health and welfare agencies and law enforcement agencies.
