Comunilife Inc. is looking for a Clinical Social Worker for its Respite program. Under the supervision of the Senior Program Director, The Clinical Social Worker be responsible for providing social work services to patients and families in collaboration with the health care team. This position is eligible to participate in Comunilife's generous benefits and paid time off package and has a salary up to $82,000 a year.

Essential Duties:
Assesses clients and/or family psychosocial status, social work needs and living conditions utilizing professional knowledge, skills of observations and interviewing skills.
Establishes the social work component of the patient/family plan of care based on goals mutually acceptable to the client, family and significant others. Makes referrals to other community services, as necessary.
Provides psychosocial work services to patient and/or family, including short-term individual counseling, community resource planning, and crisis intervention. Responds to emergent psychosocial patient and family needs, as requested.
Provides advocacy to patient and/or family, e.g., assistance in obtaining entitlements and community services.
Provides social work services in accordance with Comunilife policies, practices, procedures and Standards of Social Work Practice, which may require standing, stooping, sitting, crouching, bending and stretching to deliver patient care, as needed.
Initiates and maintains verbal and written communication according to Comunilife policy, including the preparation of clinical and progress notes, to ensure optimal quality care.
Review and acknowledge incoming referrals of potential respite applicants from Hospital Partner organizations;
Conduct person-centered comprehensive assessments of potential respite patients; including medical and support needs of the patient prior to hospital discharge;
Provide timely updates and dispositions to partner organizations on status of respite patient's acceptance in the program;
Review patient's discharge plan upon arrival and monitor on-going progress;
Facilitate and provide intensive housing case management services to facilitate client's timely placement into permanent housing. This includes but is not limited to:
Completion of Supportive Housing applications- including behavioral health consultations for psychiatric evaluations and psychosocial assessments;
Facilitate and complete 2010e housing assessments/applications;
Process assisted living and nursing home applications
Facilitate the gathering of patient's replacement document and compiling necessary documents
Facilitate and coach patients through the housing interview process
Manages social work planning for a caseload of patients from assessment to discharge. Maintains productivity sufficient to meet program goals.
Assists team members in understanding the significant social and emotional factors related to the patient’s health problems.
Participates in the development of treatment plans and revises the goals as needed. Coordinates approaches to patient and/or family care with other team members.
Consults with and educates the patient and family regarding the treatment plan, self-care techniques and prevention strategies.
Utilizes appropriate community resources and serves as a liaison between Comunilife and other community agencies.
Participates in discharge planning.
Assumes responsibility for continued professional growth, such as in-service programs.
Participate in ongoing QI and program evaluation activities
