ORGANIZATION SUMMARY

POSITION SUMMARY
The Service Coordinator is a part time position working for the Article 31 Outpatient Program. Service Coordinator provides targeted support to the wellness and recovery goals of individuals with complex and/or chronic behavioral health issues and needs by implementing targeted acute interventions designed to provide timely, high-quality, and efficient care.

Responsibilities

DUTIES, RESPONSIBILITIES, AND ESSENTIAL FUNCTIONS
• Works as part of an interdisciplinary team to ensure the coordination of resources for individuals enrolled in the Outpatient Program.
• Identifies service gaps and locates needed resources for individuals, including housing, works placement and other supports.
• Conducts routine follow up/monitoring of individuals and their services.
• Assists individuals in understanding, managing and maximizing their resources/benefits to ensure they can receive appropriate and supportive services.
• Referral to community and social support services ensuring appropriate linkages are made.
• Provides transitional care, including appropriate follow-up from inpatient to other settings.
• Provides support to people served in navigating complex health care and social services systems.
• Provides individual and family support including advocacy when needed.
• Promotes hope and recovery by using strengths-based, culturally appropriate, and person-centered practices.
• Maximizes community integration and normalization by ensuring community and programmatic resources are sought out and engaged upon.
• Consults with Clinicians to devise and manage effective ongoing care plans to at risk clients.
• Prevents, recognizes and reports abuse toward the people we support

• Ensures that all required documentation (paper and electronic) is completed /processed
appropriately, in a timely manner, and meets quality and compliance thresholds.
• Performs other duties as assigned.

Code of Conduct as well as applicable laws and regulations.

Qualifications

QUALIFICATIONS
• Education:
o High School Diploma or equivalent required; Associate Degree in Human Services field
preferred
• Experience:
o At least two years of experience working with behavioral health population
• Specialized Training, Licenses, Certifications:
o Not required, training will be provided upon hire as needed
• Knowledge, Skills, Competencies, and Abilities:
o Proficient knowledge of community resources for referrals
o Demonstrates organizational and communication skills which would enable effective
interaction with fellow team members, individuals/clients/families and the community
o Critical thinking and sound judgment skills are necessary
o Computer literacy required; familiarity with Microsoft Office Suite
o Must be efficient and well organized
• Other Requirements:
WORK ENVIRONMENT
This role is performed in-person, frequent travel throughout LI is required.
PHYSICAL REQUIREMENTS
• Employee is required to sit, walk and stand for prolonged period of time (at least 8 hours).
• Employee is required to effectively communicate both verbally and in writing.
• Employee is required to operate standard office equipment such as computers, phones,
photocopiers, fax and filing cabinets.
• Employee might need to move/lift light items.
