Location: Remote but MUST reside in or near the Western Panhandle for evaluations, events and meetings (Okaloosa, Walton, Santa Rosa, or Escambia Counties)

Due to the requirements of the state regulated position, this role does require a Bachelor's Degree.

Responsibilities

Coordinate access to health-related services by assisting clients with utilizing available resources.

Identify potential clients, complete initial needs assessment, and maintain follow-up to ensure appropriate guidance to health services.Provide informal counseling, education, and social support to clients.Assist clients with utilizing available resources, including scheduling medical appointments, accompanying clients to appointments, and assisting with completion of medical program applications.Develop and maintains strong working relationships with external (referral sources and community agencies) and internal contacts (departmental team members).Maintain accurate client records by documenting all contacts, services provided, and outcomes in order to track all clients and services for future reference.
Requirements

Education:

High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Bachelor's degree OR 4 years of applicable cumulative job specific experience required.
Additional Preferences

Due to the nature of this position one of the following is required:

Bachelor’s degree or higher from an accredited university with an emphasis in the areas of psychology, social work, health education, elementary education, healthcare administration, interdisciplinary sociology, early childhood, child development or special education, orAn out-of-field degree with a minimum of three years documented experience with young children and families in a teaching, case management or counseling role, or with individuals with special needs and/or developmental delays of any age in a case management role, orA registered nurse without a bachelor’s degree, but with a minimum of three years experience in the same areas required for those with an out-of-field degree.
