The Norfolk Community Services Board (NCSB) is the primary provider of public mental health, substance abuse, and intellectual disability services for the City of Norfolk. A continuum of services spans from outreach and prevention through intensive psychiatric care, provided by highly clinically oriented service professionals with expertise in these service areas. The NCSB is currently seeking an experienced The NCSB is seeking a Service Coordinator (Case Manager III) in their Infant and Toddler Connection division. The Infant & Toddler Connection of Norfolk is the local system of the Infant &toddler Connection of Virginia. each of Virginia's 40 localities has an early intervention program. The NCSB, is the local lead agency for our program. The Infant & Toddler Connection provides early intervention services to eligible infants and toddlers (birth through age two) who have disabilities or developmental delays. The program is mandated through Part C of the Federal individuals with Disabilities Education Act. The Service Coordinator (Case Manager III) provides service coordination for children and their families who are receiving early intervention services as designed by the Individuals with Disabilities Education Act. Support a caseload of individuals with developmental delays, atypical development and/or a medical condition with high probability of resulting in a developmental delay and their families to help the child and family access the resources necessary for the child to reach their developmental milestones. Department Hiring Salary Range: $46.589-$53,577 Essential Functions Essential functions include but are not limited to: + Work with the family from the initial intake appointment, through assessment, IFSP development, IFSP reviews, annual assessments, and transition. + Link individuals to needed resources and supports, including medical services, outpatient therapists, support groups, and preschool special education. + Contact families to check on progress monthly to determine if there are changes in status and if support is being delivered as expected. + Coordinate with service provider(s) routinely to ensure child is making progress and IFSP is in compliance with Federal and State regulations. + Document all components of work in the electronic record within program expected timelines. + Service Coordination will require travel to and work at the families' homes and the community locations. Education/Experience Bachelor's degree in a human services field. Two years experience in a human services field related to the areas of assignment. Some assignments may also require specific experience working with various types of client populations. Preferred Education/Experience: Possess a minimum of a Bachelor's degree in one of the following areas + Allied Health + Child and Family Studies + Counseling + Criminal Justice + Early childhood (including Growth, Development, and Special Education) + Human Development + Human Services + Nursing + Psychology + Public Health + Social Work + Special Education Master's Degree preferred.
