The Service Coordinator I provides individuals with a serious mental health diagnosis and who are considered chronically homeless. The primary function of Acute Service Coordination is to provide professional assessment, assist with housing, service planning and coordination, submit necessary referrals, and re-evaluate services required for a safe and healthy community life which is manifested through stability in relationships, housing, and employment. This is done in accordance with the Agency’s vision, mission, and values, and emerging mental health Recovery Principles and HUD standards. This position provides an essential recovery orientation with an emphasis on housing stability.
This position is eligible for a sign-on bonus of $3,000 for an 18-month work commitment!
Responsibilities:
Intake and Coordination
Coordinate housing and services across multiple life domains Lead and collaborate with treatment/service teams Build relationships with individuals and their support networksAssessment and Service Planning
