Service Integrator - Part-Time (20 hrs.)

As a Service Integrator, you will be responsible for ensuring that the overall needs of service recipients are met.  The Service Integrator has an understanding of Hillside’s system of care to ensure informed decisions are made surrounding utilization management throughout the system of care to include intakes, transitions, and discharges. 

This a part time position with a flexible schedule that would include at least one evening shift. 

Responsibilities:

 Screen and process referrals for services within Hillside. Utilize experience, education, age-specific knowledge and cultural competence in order to assess and support youth and families to achieve positive and successful outcomes and routinely successfully use problem-solving skills with self and others. Contact referral sources and families to collect pertinent information in order to make appropriate decision regarding appropriate services. Meet with families to gather information, answer questions and if necessary, tour programs. Act as the main point of contact for funders. Collaborate with external and internal customers, inclusive of youth and families, school districts, and treatment teams within affiliates. Ability to provide telephone triage for incoming referrals and requests for service from recipients of service, potential recipients, funders, and collaborative community service providers.  Knowledge to offer alternative community resources, act as advocate and broker, as well as assess for admission to the agency for delivery of services.  Ability to respond to emergency calls. Actively participate in agency/team meetings, volunteering and serving on community committees. Develop, maintain and update all required documentation in a timely manner.  Collect, aggregate, and analyze data to identify unmet needs and opportunities for improvement of existing services as well as determining opportunities to serve targeted populations.  Prepare forms for admissions. Data entry and form preparation. Special projects as assigned.

 

Requirements:

Bachelor’s degree in Social Work, Psychology, Mental Health Counseling, or related Human Services; Master’s preferred 1+ years of experience in youth and family centered mental health/ psychosocial treatment Valid driver’s license and meet agency driving standards

 

$18.36 Minimum Rate per hour, $27.54 Maximum Rate per hour, based on experience.
