Location Overview: This position is located at our Green House facility. GREEN HOUSE® Residences–Memory Care Homes are freestanding homes, each with state of the art amenities and 7,000 square feet of living space, uniquely designed to cater to the needs of our Elders. Our homes provide assisted living care and services under an Extended Care Congregate (ECC) license. Located on 115 acres of green space, our Elders are surrounded by the peace and tranquility that nature has to offer. Although we offer a real home setting, being a part of Bartram Campus affords us the opportunity of being part of the Bartram Lakes community. Position Summary: A Shahbaz provides care to elders living at The Green House® Residences at Bartram Park. The primary role is to protect, sustain, and nurture the elders by providing assistance with activities of daily living and facilitate a meaningful life through engagement.
Job Responsibilities:
Promotes a high quality of life and positivity towards the elders Communicates and works cooperatively with other Shahbaz's as a self-managed work team Maintains confidentiality of elder information. Helps create an atmosphere of optimism, warmth, and interest as well as calm environment Exhibits excellent service to patients, visitors, physicians, and co-workers. Shows courtesy, compassion and respect. Activity planning and participates in the engagement of the elders in their care Organizes and serves the elder’s meals Food preparation and presentation for the elders in the Green House Personal care including: showers, dressing of the elders, toileting, feeding, oral hygiene, grooming Engages the elders in daily home living activities Performs blood glucose monitor by fingerstick. Light housekeeping, cleaning, laundry care Meets with Clinical Support Team, as scheduled, to assist in identifying and correcting problems and improvement of services Transports elders to various activities, transports or retrieves equipment, specimens and other items as required. Cleans and tidies rooms, elder bathrooms, kitchen and refrigerator, utility rooms, and linen closet (removes dirty linen, clothing; straightens the elder’s personal belongings), straightens common areas, cleans other equipment or areas. Monitors (watches) elders for safety (to prevent falls/injuries/wandering) Report accidents and incidents when they occur Report any resident abuse or suspected abuse to the Guide immediately May perform other duties as assigned by the Guide and/or the Self-Managed Work Team. Maintains a clean and safe environment; identifies and reports hazards. Meets all mandatory in-service requirements. Adheres to policies/procedures specific to universal precautions when delivering patient care. Completes annual equipment and skills competency checklists. Participates in performance improvement activities as needed.Job Qualifications:
Certified Nursing Assistant License from the State of Florida required. Demonstrated experience working with the elderly population preferred.
