The Shared Living Coordinator (SLC) provides the necessary monitoring and oversight of Assistance Plus Shared Living Providers (SLP). The SLC ensures services provided by the Shared Living Provider focus on the specific needs of the member who is an adult with a developmental disability and/or autism. The SLC will ensure the quality and safe delivery of approved in-home and community services as outlined in the Support or Comprehensive Waivers, and in the HCBS Global rule.
This position will be required to meet the requirements of a regular office schedule, working five days per week Monday- Friday, between the hours of 8am and 5pm.
Because the SLC/ will drive to a client’s home in order to monitor services, proof of vehicle insurance at $50K/$100K and a valid Maine driver’s license are required.
Duties include and are not limited to:
Recruiting and retaining Shared Living Providers Ensuring the completion of reference and background checks completed by HR Participating with the Case Manager (CM), SLP, member, and/or Guardian in the matching process Conducting home visits to assure the Provider has the skill set and lifestyle to support the individual Entering data into Enterprise Information System (EIS) for all reportable events related to members Documenting home visits, phone contacts, and consultations Ensuring that the client’s plan of care drives all service activities Maintaining regular contact with the SLP according to contract and MaineCare standards Conducting home care compliance visits to assess compliance with health and safety codes, behavioral regulations and behavior plans Assuring the completion of progress notes and medication administration reports by the SLP Conducting home visits every other month at minimum alternating with every other month phone calls, member must be present for 2 documented visits annually Ensuring the completion and accuracy of daily documentation per MaineCare benefits manual Completing the Shared Living Home Visit (SLHV) tool annually coordinating with CM to alternate completion every 6 months Providing CM with and receiving from the CM copies of the SLHV Reporting to CM, OADS, and Guardian issues with medication administration, documentation, and significant issues impacting the member’s support Partnering with CM to coordinate services and share information Ensuring the execution of a contract annually with the SLP according to regulations Providing or offering available trainings to the SLP for free if available Ensuring the SLP has completed trainings required of the contract Attending MACSP Shared Living and CSP meetings, along with other trainings provided by OADS consistently to grow knowledge of services Following and meeting strategic planning requirements Ensuring SL Providers comply with Home and Community Based Service (HCBS) regulations Promoting HCBS guidelines around the promotion of competitive integrated employment Updating SL Operations manual as needed to meet HCBS regulations and comply with agency updates Carrying a minimum caseload of 20 contracted Shared Living cases with expectation to continue growing the program continuously Performing other duties as assigned
