Veterans Inc., the largest provider of services to veterans and their families throughout New England, is looking for a professional, highly motivated, goal-oriented, skilled, and hardworking individual to fill the Site Coordinator position and become a valued member of Veterans Inc. team!
This individual would be responsible for overseeing the care and rehabilitation to veterans living within a residential facility, and will provide clinical direction to residential staff and program participants. This individual would be based out of our Bradford, VT office.
WHAT YOU WILL BE RESPONSIBLE FOR DAY TO DAY:
+ Provides clinical management and oversight to staff and clients of residential housing program for previously homeless veterans.
+ Manages the day to day operations of the designated site.
+ Collaborates with the Admissions Manager to conduct initial intake and evaluation of applicants. Interviews potential residents and/or referral source regarding appropriateness. Secures all pertinent forms for individual admission (e.g.: DD 214, PPD test, Psych/Soc Evaluation, Cori Checks).
+ Provides clinical team leadership for Residential Advisors and/or Residential Case Managers working on site, including providing and supervising counseling sessions to clients.
+ Provides clinical guidance to staff and makes recommendations and/or referrals to other services or programs that will properly meet individual needs.
+ Prepares and maintains case records, progress notes, narrative reports, statistical reports, case conference reports, or other documentation as required, and maintains ongoing quality improvement of staff's clinical records.
+ Assess and collects monthly program fees, and assures staff compliance with collections.
+ Performs daily inspections of housing units.
+ Reports any equipment/operational problems to the Director of Operations and or Facilities Manager for repair.
+ Supervises snow removal on site. Supervise spring/fall clean up and cutting of the grass in the summer for the site.
+ Conducts House Meetings on a weekly basis.
+ Performs random Breathalyzer and Urinalyses as needed.
+ Attends and actively participates in all Veterans Inc. meetings and programs as required.
+ Responsible for Quarterly Performance Measures to be reviewed by supervisor.
+ Other duties and special projects as assigned.
WHAT YOU MUST HAVE:
+ Minimum of Bachelor's degree in Human Services, Social Work or related field.
+ Minimum of three (3) years' experience in program management, case management, substance abuse prevention, outreach, and/or services to the homeless or veteran populations.
+ Must have strong computer skills and knowledge of Microsoft Office Suite.
+ Must have a valid driver's license, and be unopposed to travel.
+ Knowledge of and previous experience using Homeless Management Information Systems (HMIS) a plus.
+ Established community connections and knowledge of veterans' benefits are both highly valued
+ Must be able to pass a CORI and background investigation.
+ Compliance with Veterans Inc. COVID-19 Vaccination Policy. Religious and Medical Exemptions are considered.
