**Practice Description** **Fairview** is seeking a **Site Medical Lead - Internal Medicine / Pediatrics Physician** that will serve as the front-line provider lead in the **M Health Fairview Clinic - Cottage Grove** location that partners with the Clinic Manager, Clinical Supervisor and providers at that location to meet day-to-day patient care needs and support execution of key projects. Assures the coordination, direction, and collaboration of services and resources related to the discipline as well as achieving productivity, financial and operational goals. **Job Details:** + **Opening: 1.0 - .75 FTE** + **13 provider primary care** practice. + Patient encounters average **18-22** **patients per day.** + **Call Frequency: approximately 2 weeks per year - phone consultation only with first tier nurse triage** + As a Pioneer Accountable Care Organization (ACO), our physicians use a **multidisciplinary team approach** in managing clinical care. + **Fully integrated** Electronic Medical Record (EPIC). + Serves as first point of contact to address front line provider or patient issues. + Works in partnership with Clinic Manager and Clinical Supervisor to address issues and escalate them to the Medical and/or Operations Director as needed. The actual rate of pay offered within this range may depend on several factors, such as FTE, skills, knowledge, relevant education, experience, and market conditions.
