POSITION PURPOSE
The Trinity Health Alliance of MI (THAM) Skilled Nursing Facility Coach is a highly visible position within THAM High-Performing Network of Skilled Nursing Facilities. The incumbent has the responsibility of training, educating, and working with SNF staff to re-engineer processes and procedures relating to improving health outcomes, patients’ experience of care and SNF efficiencies in support of high-quality care and the High-Performing Network SNF scorecard. The SNF Coach is dedicated to working with SNF Administration and THAM staff to improve performance on key outcomes which drive consistent achievement of the Quintuple Aim goals for THAM attributed lives.
The colleague, by working with SNFs and analyzing available data, will identify opportunities for improvement and develop and implement action plans with a goal of helping the SNFs improve performance relative to key metrics and efficiencies. This position has the responsibility of maintaining relationships with SNF administration and their staff. It is imperative that the SNF Coach provides initial and continuous service and support offered education to SNFs. The success of this position will increase efficiency as the High-Performing Network strives to comply with organizational guidelines and the Clinical Integration programs. Quality service demonstrates the value of being a High-Performing Network member, therefore, increasing SNF participation and ultimately THAM's position in the marketplace.

ESSENTIAL FUNCTIONS
1) APM Driven Outcomes/Assist SNFs in attaining High Performing Network status.
a) Support THAM Performance Initiatives by participating in process improvements to support total cost of care/quality outcomes, including data collection and SNF chart audits.
b) Cascade relevant information to the SNF leaders for staff awareness about clinical initiatives, quality programs, in alignment with High-Performing Network SNF scorecard.
c) Inform SNF administration and staff about clinical integration initiatives and the importance of participating and meeting all the recommendations and requirements in all the various areas.
d) Explain the SNF report card criteria and results to SNF administration and staff.
e) Contribute to summary SNF presentation materials.
f) Completes SNF site audits and maintains database of results and tracking mechanisms.
2) High-Performing Network Engagement Functions
a) Hold regular coaching sessions, typically face to face, with identified Network SNF members to help them meet various goals, share performance data and results, and jointly develop improvement action plans. (Coaching each SNF on their overall performance is in reference to High-Performing Network SNF Scorecard).
i) Coaching sessions (at times in conjunction with Post-Acute Care Coordinator (PACC)) may include the following topics: reviewing SNF performance results, using Risk tools to identify high risk patients, tactics for improving clinical results or efficiency, tactics for avoiding ambulatory sensitive re-admissions, and collaboration with SNFs to address overall length of stay monitoring. Perform Population Root Cause Analysis (RCA) on readmitted patients and prepare reports, ensure disease management protocols are implemented and followed and periodically provide feedback to SNFs on statewide criteria.
ii) Provide the SNF administration with specific recommendations on improvement action plans and jointly decide on next steps, including the next period for feedback on performance. Use data to determine educational opportunities.
iii) Demonstrate ability to describe all components affecting performance.
3) Monitoring Performance Metrics
a) Develop and utilize appropriate monitoring and feedback tools and/programs for work plans and actionable items discussed in SNFs. Support development of materials supporting outcome improvements.
b) Implement monitoring tools, including declined referrals.
c) Support the development of tools to assist SNFs in meeting various High-Performing Network goals, including educational materials for SNFs and patients, email alerts and reminders, as well as others to be determined.
4) High-Performing Network Activities
a) Assists SNF staff in obtaining applicable accounts, passwords, administrative rights for the various THAM systems. (e.g. Epic CareLink)
b) Educate SNF staff on Trinity-Health Network.
c) Coordinates, conducts, and provides education and training orientations to new and existing SNFs. Identifies and promulgates best administrative and clinical practices across SNFs.
5) Support other projects as directed
a) Maintains a working knowledge of applicable Federal, State, and local laws and regulations, Trinity Health’s Integrity and Compliance Program and Code of Conduct, as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical, and professional behavior.
b) Other duties as assigned.
MINIMUM QUALIFICATIONS
1. Bachelor's Degree or higher in healthcare and /or healthcare administration or an equivalent combination of education experience in post-acute operations, or customer support.
2. Ability to develop proactive partnerships SNF administration and staff, peers, senior leaders and other stakeholders.
3. Highest level of interpersonal and business communication skills (verbal and written) to interact and influence people at all levels within the organization. Demonstrated ability to effectively communicate and present information to senior management, and medical staff leadership.
4. Ability to problem-solve and execute initiatives. Ability to use judgement to determine steps to resolve issues and involve others appropriately.
5. Sensitivity to SNF issues and time constraints. Adheres to high standards of follow-through on issues.
6. Proficiency with or ability to learn clinical information systems. Advanced knowledge of Microsoft Office Products required.
7. Must be comfortable operating in a collaborative, shared leadership environment.
8. Must possess a personal presence that is characterized by a sense of honesty, integrity, and caring with the ability to inspire and motivate others to promote the philosophy, mission, vision, goals, and values of the RHM.
PHYSICAL AND MENTAL REQUIREMENTS AND WORKING CONDITIONS
Must be able to set and organize one's own work priorities and adapt to them as they change frequently. Must be able to work concurrently on a variety of tasks/projects in an environment that may be stressful with individuals having diverse personalities and work styles.
2. Must be able to travel locally to the Skilled Nursing Facilities. (minimum 50% of time).
3. Must possess the ability to comply with RHM policies and procedures.
