The role ensures timely identification of eligibility and referrals to appropriate services. This position plays a critical role in ensuring individuals receive access to the appropriate level of care. **Essential Job Duties:** - Conduct initial screenings and assessments for program eligibility using standardized tools and guidelines under the 1115 Waiver framework. - Collect and document accurate demographic, medical, and financial information to determine eligibility. - Collaborate with individuals, families, and caregivers to gather necessary documentation. - Maintain up-to-date knowledge of 1115 Waiver eligibility requirements, program rules, and available services. - Provide referrals to Navigators or other support staff for further case management or service coordination. - Maintain accurate and confidential records of screenings in compliance with program policies and HIPAA regulations. - Communicate screening outcomes and next steps to applicants in a compassionate, professional manner. - Identify and address barriers that may prevent individuals from completing the screening process. - Assist in program outreach and awareness activities to connect underserved populations to services. - Any other duty as assigned by your Supervisor. **Skills:** • Strong communication and interpersonal skills. • Ability to work independently and manage multiple tasks efficiently. • Proficiency in Microsoft Office and database management systems. • Bilingual skills (if applicable) are highly desirable. **Minimum Education Required:** Associate or bachelor’s degree in social work, public health, or a related field preferred. **Minimum Experience Required:** One year experience in eligibility screening, social services, or healthcare preferred. **License/Registration/ Certifications Required:** NYS Driver’s License **Training Requirements (paid trainings after hire):** - New Hire Orientation - All 1115 Screener trainings by WNYICC **Physical Requirements:** The overall nature of the position is sedentary requiring little physical effort with occasional light physical exertion required.
