Ebenezer offers a robust compensation package with generous benefits and wages ranging from $14.21 -  $20.00 per hour. Wage determinations will be based on similar experience.

 

Ebenezer is looking for a Social Service Assistant to join our team! This role is responsible for assisting the Social Services department in meeting the needs of residents and families through various tasks and activities all completed under the supervision of the Social Service Director at our long term care facility in Burnsville, MN. Responsibilities/Job Description Advocates for resident’s rights and needs  Assists with resident room changes  Follows up and assists residents with missing items  Scans psychology notes to Medical Records · Helps completes admission paperwork  Follows up on grievances Coordinates Resident and Family Council Coordinate community referrals and resources as needed  Ordering residents clothing and other personal effects  Helps residents with Medicaid applications as needed  Notification and correspondence with families as neededFacilitates and assists residents with completion of electronic satisfaction survey Qualifications

Required:

0-1 years of experience

 

 

Preferred:

High School Diploma/GEDStrong verbal and written communication skillsAbility to use and utilize various forms of technologyAbility to maintain high level of confidentiality2 years of experience in LTC field
