Our Social Services Assistant do an amazing job with assisting in the identification of medically related social services as well as supporting the overall well-being of our residents, including the researching appropriate services.

Responsibilities The Social Services Assistant provides support and advocates for resident and family.The Social Services Assistant participates in discharge planning, development and implementation of social service care plan.The Social Services Assistant assists in vulnerable adult investigations. Qualifications Experience in admission/discharge procedures preferredHospital or nursing home experience preferredAble to read, write and communicate in English
