UMC is seeking a Social Services Coordinator for Covenant Place.
Covenant Place a UMC Community, is a senior affordable housing community located close to shops and public transportation in Plainfield, NJ, is seeking a Social Service Coordinator.

The Social Services Coordinator links residents and non-residents to supportive services in the community, assesses needs, and eligibility for entitlements. Works closely with management to ensure that lease obligations are fulfilled.
Requirements for a Social Services Coordinator:
+ HS Diploma or equivalent.
+ Associate's degree or some college coursework required ; significant work experience may replace college level work.
+ 3 years of experience in services for the elderly, disadvantaged or social services in a health care or related setting.
Responsibilities for a Social Services Coordinator:
+ Provides assistance to residents and home-based non-residents by connecting them to supportive services. Assists with placement to higher levels of care and assists families and agencies in this process as needed. Establish resources and resource guide.
+ Monitors the frailest residents through regular contact and post hospital/therapy transitional care.
+ Creates communication vehicles such as a newsletter, website, social media, marketing material and in a liaison function to resident groups.
+ Assures socialization needs of residents are met through arranging events, classes, spiritual resources, vocational and education supports and congregate services.
