Position Overview As the Social Services Director, you will evaluate, oversee, and direct the social services department to ensure residents receive the highest quality of medically related social work services so that each resident may attain or maintain the highest practicable level of physical, mental, and psychosocial well-being. You will collaborate with our interdisciplinary team to create individualized care plans and provide both residents and their families with the support they need during their time at our facility. Key Responsibilities Conduct comprehensive psychosocial assessments for behavioral and mental health patients. Develop individualized care plans and participate in interdisciplinary care conferences. Advocate for residents’ rights and ensure their psychosocial, emotional, and mental health needs are met. Facilitate family mediation, support, and de-escalation in times of crisis. Oversee the admission, transfer, and discharge planning process, ensuring a smooth transition for residents. Coordinate with community resources, including hospice care, mental health services, and substance abuse programs. Maintain compliance with federal, state, and local regulations through accurate documentation and daily regulatory rounds. Assist with end-of-life care planning, including hospice referrals and advanced directives. Participate in quality assurance and regulatory compliance initiatives to improve resident care outcomes. Qualifications Bachelor's or Master's degree in Social Work or a related human services field. Licensed Social Worker (LSW) or Licensed Independent Social Worker (LISW) credential in the state of Ohio. Experience in a skilled nursing facility, behavioral health, or substance abuse treatment setting preferred. Strong communication and interpersonal skills, with a commitment to resident advocacy. Knowledge of federal, state, and local regulations related to long-term care.
