What you'll do The primary purpose of the Director of Social Services is to plan, organize, develop the Social Services Program in accordance with current applicable Federal, State, and local standards or regulations. This will assure residents receive the highest degree of care while their social, medical and emotional needs and rights are met. The Social Worker shall help the residents, staff and families with problems that retard or prevent the resident’s social adjustment or any other problem that affects the resident’s ability to benefit from his/her stay with the facility. The Director of Social Services will be involved with the In-services Education. He/she will teach the staff how to recognize the problems and needs of the residents and help the residents take appropriate action in relation to these problems. The Director of Social Service will serve as a committee member for the following committees: 1. Management 2. Admissions 3. Quality Assurance 4. Patient Care Policy 5. Dispute Mediation Committee Provides oversight to operations of the department and staff. Plan, draft, review and implement policies on Social Services in the facility with the consent of the Administrator. Supervise Social Worker Assistant and Secretary. Participates in various committees as directed by the Administrator/CEO. Functions as the social worker involved in the pre-admission process and committee. Receives and maintains all referrals for Skilled Nursing Facility. Maintain routine contact with primary referral sources. Complete all admission packets and required documentation with residents and/or designated representatives on admission as needed. Provide information and tours for placement inquiries. Perform discharge planning activities as need indicates. Act as liaison for the facility with residents, designated representatives and the community services. Provide supportive counseling and referral as needed. Consult with clinical staff regarding the resident’s psycho-social needs. Provide in-service education. Attend workshops and seminars to remain current in the provision of social work services. Meets regularly with consultant. Maintain contact with other social workers in Long Term Care. Acts as staff liaison to the established Resident Council. Perform other duties as required. Act as liaison with facility Ombudsmen. Coordinate Family Council. Assist with all complaints and grievances Attend all required in-services and follows department and facility policies and procedures including the use of personal protective equipment. Maintain CONFIDENTIALITY of all pertinent resident care and employee information and follow HIPAA and HIV regulations. SOCIAL WORK DUTIES AND RESPONSIBILITIES: Direct and advise other Social Workers with duties listed below and cover for other social workers in their absence. Function as a member of the multi-disciplinary team. Complete a care plan for each resident and update as needed. Provide written documentation in the medical record at least quarterly and as changes occur. Is responsible for welcoming all new admissions and complete necessary documentation in a timely manner. Notify resident and/or designated representative of all changes in status, transfers, incidents, etc. Coordinate acquisition of personal items for residents (i.e., clothing, eyeglasses, and hearing aids, etc.). Maintain appropriate office records. Coordinate discharge planning with resident, designated representative, team members and appropriate community service agencies as needed. Assist residents and/or designated representative with financial concerns (Medicare, Social Security, Medicaid). Provide residents and/or designated representatives with information and education regarding Advance Directives at time of admission and at least annually. Complete and distribute notices and changes as needed when department secretary is unavailable. Complete duties of departmental secretary in her absence. Perform other duties as assigned by the Administrator. Attend all required in-services and follow all department and facility policies and procedures including the use of personal protective equipment. Maintain CONFIDENTIALITY of all pertinent resident care and employee information and follow HIPAA and HIV regulations. What we need from you Education: Master's Degree in Social Work, required Experience: Minimum 2 years experience as a social worker in long term care, preferred Licensure/Certifications: Licensed Master Social Work, preferred Skills: Accountability Analysis & Evaluation Communication Customer Service Leadership Reliability Time Management Physical Requirements: Light Work - Exerting up to 20 lbs of force frequently, and/or a negligible amount of force constantly to move objects. If the use of arm and/or leg control requires exertion of forces greater than that of sedentary work and if the worker sits most of the time, the job is considered light work. Occasional balancing, climbing, crouching, grasping, lifting, pulling, pushing, reaching, receptive motion, standing, stooping, walking Frequent typing/clerical/dexterity, seeing/monitor/computer use, talking, visual acuity Hazards: Occasional bodily fluids/bloodborne exposure Travel: Occasional Travel - The job may require travel from time- to-time, but not on a regular basis. Pay Range Salary is based on a variety of factors, including, but not limited to, qualifications, experience, education, licenses, specialty, training, and fair market evaluation based on industry standards. The above compensation range represents a good faith belief of the compensation range by Bassett Healthcare Network, and its entities and affiliates, at the time of this posting or advertisement. Pay Range Minimum: USD $68,640.00/Yr. Pay Range Maximum: USD $79,040.00/Yr.
