Provides direct social work services to hospice patients and their families according to assessed needs and the individualized plan of care, in conjunction with the hospice team. Also provides social work service for home care patients of Riverways Home Care. Participates in bereavement and community education programs. Master’s degree in social work from accredited school of social work
