Social Worker, Inpatient (NE)
Summary: Under the Direction of the System Manager of Inpatient Behavioral Health Social Work, the Inpatient Social Worker will provide direct services to adult psychiatric patients on the acute behavioral health units at CCHP, including leading groups, individual therapy, family and systems meetings, completing intakes, case management activities, and participating in crisis management as needed. The Social Worker is an integral member of the multi-disciplinary treatment team. The Social Worker participates in interdisciplinary treatment planning; completes psychosocial evaluations of admitted patients and meets the documentation and turn-around-time standards. The Social Worker facilitates the discharge planning process on day one of treatment and maintains communication with collateral resources. The Inpatient Social Worker maintains patient charts appropriately, including accurate documentation of assessments, coordination of care efforts, discharge planning efforts and other interventions throughout the patient’s episode of care. The ideal candidate will be a team player, have excellent organizational skills, effective communication skills and the ability to work well within a multidisciplinary disciplined environment, maintaining positive relationships with physicians, patients, families and all other hospital employees. Education: Broad knowledge of social work practice, Master’s Degree in social work, Mental Health Counseling or Psychology from accredited programs, RI Board Certified Alcohol and Drug Profession Licensure: Master’s Prepared or LCSW/LICSW/LMHC/LCDP Experience: One – two years of clinical work in acute care settings recommended. Skills and Abilities: · Knowledge of psychiatric/medical social work or related clinical field. Requires judgement to analyze facts and circumstances and to make independent discussions abased on limits of standard or accepted practice. · Ability to effectively use clinical and therapeutic skills. · Knowledge of and experience with Evidence Base Practice in Group Facilitation · Skills in organization and priority setting. · Ability to work cooperatively with peers and other disciplines. · Ability to utilize sound interpersonal skills when interacting with patients and taking into consideration cultural background, linguistic skills and age of customers.
