Social Worker, Licensed - LTC

SUMMARY

The Licensed Social Worker is responsible for implementing resident services for residents consistent with regulations and established best practices. The Social Worker provides support through resident/family advocacy, ongoing assessment and evaluation, service coordination and care planning. The primary focus of this position is the quality of life of residents to assist them in achieving their optimal psychosocial, emotional and behavioral well-being. This position reports to the Campus Director of Social Services.

 

PRIMARY RESPONSIBILITIES

Manage a range of services to residents, tenants, clients, and their families (hereafter referred to as “residents”); including Admission, Discharge, Transfers, and ongoing services through the duration of a resident’s stay.*Ensure a welcoming admission process by providing emotional support, information regarding day-to-day activities, conducting tours, and encouraging socialization for the resident. Assist in creating and maintaining an environment that achieves high quality care (physical, mental, emotional, social and spiritual) of residents and maintain their independence and dignity to the highest degree possible. Establish and maintain effective communication systems with all customers and respond promptly to resident and family issues.*Coordinate the IDT (interdisciplinary team) to ensure adequate and appropriate flow of services. Promote the resident’s well-being and satisfaction by supporting person-centered care in all areas of the resident’s life. Develop and interpret standards of care, guide the interdisciplinary team to establish and implement the resident’s plan of care with the resident and his/her family and the interdisciplinary team. Help improve systems and eliminate barriers to resident centered care and services*Coordinate ongoing assessments to manage changes in Clients’ physical and emotional needs.*Determine scope and length of services required on a case-by-case basis.*Plan, implement and monitor the provision of resident services on your caseload. Provide service coordination for residents to include arranging and/or referring appropriate support services. Plan interventions based on assessments and regularly reassess and revise treatment plans in response to newly identified needs and altered goals of care. Demonstrate and apply knowledge of the resident/family dynamics involved in aging, grieving, and age specific human growth and development. Demonstrate and apply knowledge and understanding of mental and emotional dysfunction.Implement and follow established resident services policies, procedures and practices. Develop, Implement and maintain monitoring systems and processes to ensure compliance with regulations and established best practices. Apply the knowledge and skills needed for the position, including technical competence in the field and current knowledge of laws and regulations affecting your position. Maintain current knowledge of community resources available for residents. Use discretion and independent judgment to compare and evaluate possible courses of action and decision-making. Maintain knowledge of organization and industry policies and practices.Maintain accurate Social Services records and Medical Records to ensure compliance with regulatory and organization requirements.*Facilitate discussion with resident and families regarding Advance Directives and Providers Order for Life-Sustaining Treatment (POLST).  Complete periodical reviews of documentation. *Interact with internal departments pertaining to care and financial issues.*Complete the necessary documentation per regulation and Sholom policy and procedure including the Brief Interview for Mental Status (BIMS) and Patient Health Questionnaire (PHQ-9) as required. *Serve as a resource for residents and families, providing ongoing support and advocacy. *

Establish and maintain effective working relationships with outside agencies and organizations to promote the campus and services, and to participate with the various community constituencies to achieve mutually beneficial results. Coordinate resident services collaboratively with other professionals inside and outside Sholom. Promote cooperation with all regulatory organizations managing with an expectation of appropriate compliance and corrective measures. Refer questions outside area of expertise to appropriate team members or community agencies.

Facilitate meetings pertaining to conflict resolution, concerns, bereavement services, and group counseling.*Develop and maintain relationships with other agencies and community resources to ensure ease when directing referrals.*Support various other Social Services responsibilities as needed.


JOB SPECIFICATIONS

Education, Experience and Certifications

Bachelor’s degree in Social Work, Gerontology, or related field required.  An equivalent combination of professional experience and education considered at discretion of management.  Licensed by the MN Board of Social Work and maintain unrestricted licensure.One or more years of social work experience preferred, preferably in a long-term care environment.Transitional Care Unit (TCU):  One or more years of social work experience preferred, in a long-term care setting.Medicare Class A Hospice:  CPR Certification required.

Display high levels of customer service, responding promptly and thoroughly to the inquiries and needs of individuals both internal and external to the organization.Familiar with available community resources.  Able to establish cooperative referral relationships with other agencies.Excellent interpersonal skills to effectively build relationships, exhibit empathy, handle interactions at all levels, and respond appropriately in sensitive situations.Capability to work with highly emotional and stressful situations.Understand and maintain confidential nature of organization and client information, including and in accordance to HIPAA regulations.Ability to effectively set and maintain boundaries when working with clients.Possess effective problem solving skills.Strong computer skills including Microsoft applications such as Word, Excel, and Outlook.Knowledge of state and federal laws related to mental health and government programs.Able to display high levels of customer service, responding promptly and thoroughly to the inquiries and needs of individuals both internal and external to the organization in accordance with our PROUD standards.Able to plan, prioritize, coordinate, and manage own work.   Able to work unsupervised, make decisions and solve routine problems independently, effectively and creatively.Capacity to work with a team of interdisciplinary professionals, including Nurse Leaders, Therapy, Therapeutic Recreation, and Nutrition Services.

 

PHYSICAL REQUIREMENTS NECESSARY TO PERFORM THIS JOB

Incumbent must be able to:

Perform light office work, with occasional or negligible lifting up to 10 pounds.Push/pull Clients in wheelchairs up to 200 pounds a distance of several feet.Effectively communicate in the English language at a high level, both verbally and in writing, with people inside and outside the organization.Work at a desk for extended periods of time, including performing work at a computer.Move around the organization office repeatedly throughout the day.Apply manual dexterity, visual acuity and hearing ability for phone/computer use, office equipment use, reviewing detailed reports, information, and fine print.Work a flexible schedule when needed, including occasional evening, early morning and/or weekend hours as required for events, meetings, etc.

WORK ENVIRONMENT

The work environment is primarily indoors. Occasional exposure to the outdoors and inclement weather when traveling between sites.
