**Employment Type:** Full time **Shift:** **Description:** Home visiting services to pregnant and post-partum moms. This is a collaborative grant position with Nationwide Children's Hospital. **Social Worker, LISW** _This is a grant funded position._ **Position Purpose:** Provide health education to pregnant and parenting women at high risk for poor birth outcomes through visits to their homes. Reduce risk from maternal depression, tobacco use, drug/alcohol use, late prenatal care and unsafe sleep environments. Empower community members to be an active part of their health care. Help to improve access to health care and health care insurance. Provide support and linkage to available community resources and help to remove social determinants of health barriers to help our families achieve their goals.
