Social Worker – Emergency Department

Job Summary:
In this position, the Emergency Department Social Worker provides comprehensive psycho-social assessment, management, and support to at-risk patients at both the pediatric and adult Emergency Units, at the Downtown Campus and offsite support at the Community Campus during overnight hours. Knowledge of social determinants of health and drivers of utilization are preferred. Duties will include consultation through assessment, brief counseling, intervention, planning, and referrals to meet the psycho-social needs of patients and families. Candidate shall learn to initiate and implement advance directives. The Social Worker will actively manage patients through multidisciplinary collaboration, coordination, assessment, identification of barriers to care, and referrals for services to meet individual health needs. Candidate must possess strong written and verbal communication skills, have knowledge of available community resources, and participate in departmental meetings/in-services, education, and quality improvement activities. Upstate Social Workers perform other relevant duties as assigned including back-up coverage and support for other social workers to meet operational needs.
Minimum Qualifications:
M.S.W from a Social Work program accredited by the Council on Social Work (CSWE); M.S.W., L.M.S.W., L.C.S.W. or eligible, 3 years relevant post-master's experience (preferably in a hospital and/or healthcare environment) required. New M.S.W. graduates who are L.M.S.W. eligible will be considered for appointment to entry level (SL2) position at a commensurate salary after obtaining a Limited Permit. Incumbent must obtain L.M.S.W. within 12 months of receipt of limited permit.

Work Days:
Overnights 11pm-730am with every other weekend rotation. Includes occasional on-call/holiday coverage on a rotation based on operational needs
