Waterview Heights Nursing and Rehabilitation is currently seeking a Full-Time Social Worker.

Salary: $45k-60k per year

Duties Include:

Interviewing participants and their families

Provide psychosocial assessment and counseling to participant and families.

Developing specific care plans based on the needs of individual patients.

Helping participant apply for appropriate social services.

Providing crisis intervention as needed

Providing grief and loss counseling and supportive counseling

Strengthening communications among participant, families, and the program or facility staff

Requirements Include:

A current/valid bachelor's degree

Long Term Care exp. is a plus.

NYS Screen Certification preferred.

Strong admission and discharge planning experience
