Position Summary

Social Workers (BSW) provide psychosocial assessments, early interventions and support for people going through all kinds of difficulties in life by helping patients in developing skills to cope with the impact of illness and continuing care issues.

Complete and maintain documentation in a timely and accurate manner according to federal, state and center guidelines Serves as an expert, acts as a liaison between patients and community resources. Develop service plans and conduct reviews as needed throughout the duration of treatment. Monitor referrals made to community-based organizations, medical care and other services to support the members overall care management plan. Collaborate with a variety of health professionals in the patients circle of care including but not limited to primary care providers, care nurses, care coordinators, community health program workers, and other specialists in weekly case review meetings. Provide group interventions using evidence-based techniques such as behavioral activation, problem-solving treatment, motivational interviewing, or other treatments as appropriate. Connect patients and family members with community and social resources. Assesses and intervenes in situations involving child or elder abuse/neglect, domestic violence.
