Responsibilities Assesses the social, psychological, cultural, environmental and financial situations concerning the patient and family. Act as a patient/family advocate, promoting rights and dignity and striving to involve the patient/family in aspects of care as appropriate. Facilitates the acute, rehabilitative, and long-term discharge processes to provide for an optimal age-specific continuum of care for the populations listed below. Qualifications Job Requirements: High School (or GED equivalent) diploma. Must be a Licensed BSW within one year of employment. Preferred Job Requirements: Licensed Social Worker with two years of experience, preferable in a medical setting.
