**Position Summary**
This Social Worker role is with the Costco Team and is fully remote; however, candidates must reside within 45 minutes (reasonable driving distance) from the local office located at 7034 Alamo Downs Parkway, San Antonio, 78238.
Normal Working Hours: Monday through Friday 8:00am-5:00pm CST. No weekends or holidays will be required.
Candidate may be required to go to the office for any IT issues once hired. Travel to the San Antonio office may be required for quarterly meetings or PRN meetings.
The Social Worker role a licensed Social Worker position. This role partners with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member’s benefit plan and/or health needs.
+ Responsibilities include researching available options and resources for members with financial, transportation, safety, and housing needs. Population spans the entire lifecycle.
+ All member contact is done telephonically or via electronic exchange.
+ This position works to empower members to make positive lifestyle and medical choices and works as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health.Utilizes clinical judgment and critical thinking skills to help member’s achieve maximum physical and behavioral health through assessment and care planning, direct provider coordination/collaboration to promote effective utilization of available resources resulting in optimal, cost-effective outcomes.
+ Utilizes motivational interviewing skills to ensure maximum member engagement. Provides coaching, information and support to the member and the case manager assisting to empower the member to make ongoing independent medical and lifestyle choices.
**Required Qualifications**
+ Must have active, current and unrestricted LCSW or LMSW and AWSB exam score
+ Must be willing to obtain additional state licenses will be required upon hire
+ 3+ years of direct clinical practice experience post master’s degree, e.g., hospital setting or alternative care setting such as ambulatory care or outpatient clinic/facility.
+ 1+ years of MS Office suites experience
+ Must reside 45 minutes 45 minutes (reasonable driving distance) from the local office located at 7034 Alamo Downs Parkway, San Antonio, 78238 and be able to attend quarterly meetings or PRN meetings per business needs
+ Must be willing and able to work Monday through Friday 8:00am-5:00pm CST. No weekends or holidays will be required
**Preferred Qualifications**
+ Managed care/utilization review experience preferred.
+ Case Management and discharge planning experience preferred
+ Bilingual preferred
**Education**
Master’s degree in Social Work
**Pay Range**
The typical pay range for this role is:
$46,597.20 - $101,228.40
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
