Social Worker, Walton Medical Office United Health Services is seeking a Social Worker to join our Walton Family Practice 09 department at Walton Medical Office in Walton, NY. This position is benefits eligible. Shift: Day Hours per week: 40 Salary range: $25.74 - $38.61 per hour, depending on experience. Social Worker / Overview This position provides psycho-social evaluation, crisis and trauma counseling, resource referral and case management services. Accountable for intervening with patients and families by applying critical thinking skills to monitor psycho-social status and issues that impact the clinical progression. Includes the facilitation of patient and family communication, coping and decision making. Works as a part of a multidisciplinary team Key Responsibilities and Expectations Education/Experience Minimum Required: + Master's Degree in Social Work (MSW) with 1 year experience in a healthcare setting Preferred: + 3 years post graduate experience License/Certification Minimum Required: + NYS Licensed Master Social Worker (LMSW) + Medical Social Work from School of Social Work accepted by Council on Social Work Education (ASSW, CSW) + CPR Certification Work Environment + Majority of work is performed in the patient home. Work in the office requires working in close quarters with other employees. The offices are air conditioned/heated. Gloves, masks, gowns, other personal protective equipment are used, per blood and body fluid and isolation precautions.
