The Social Worker facilitates organized and integrated patient services during the acute phase of care for patients. The Social Worker provides ongoing assistance and consultation through comprehensive assessment, planning/implementation, communication of individual patient needs and discharge planning. The overall goal of the position is to enhance the quality of patient management and patient satisfaction, and to promote continuity of care and cost-effectiveness. Works in partnership with Care Management Team. Works in a close and collaborative role with physicians and health care professionals who are providing care and service. Works closely with patients and their families. Has frequent contact with office staff of physicians, plus other community providers.
Qualifications:
A. Education:
Bachelor’ degree in Social Work required; Texas license preferred.
B. Training and Experience:
A minimum of one-year experience in an acute care setting is preferred. Excellent verbal and written skills, well-rounded critical thinking skills, self-motivated, flexible, and goal-oriented. The ability to tolerate frequent interruptions and demanding work load.
C. Job Knowledge:
Demonstrates knowledge of the principles of growth and development over the life span and possesses ability to assess data reflective of the patient’s status and interprets the appropriate information needed to identify each patient’s requirements relative to his/her age specific needs.
