**Job Requirements** **Description:** Medical Social Worker - Case Management Womens Health + Saint Francis Hospital and Medical Center is looking for **LCSW / LMSW Social Worker** to join our Case Management team. The Social Worker is responsible for assessing and providing direct Social Work services and interventions to patients and families in the hospital setting upon admission and throughout discharge.
