Responsibilities
As an LCSW in Care Coordination you are responsible for performing social work assessments and interventions for patients.
Essential Duties:
Plan for discharge and continuity of care.Responsible for advocacy and education of patients and families.Responsible for crisis intervention, psychosocial assessment, counseling and support.Facilitates patient decisions and communications regarding post acute care.Provides support and social work to outpatients as needed.Shift: Tuesday - Friday 8am - 6:30pm

License/Certifications:
State of Oregon Licensed Clinical Social WorkerA Clinical Social Worker Associate working towards Clinical Social Work licensure with 1 year of experience will also be considered. State of Oregon Licensed Clinical Social Worker required within 2 years of hire.Experience:
2 years post MSW experience5 years post MSW experience preferredEducation:
Masters degree from a school of social work accredited by the Council of Social Work Education
