**Responsibilities:**
GENERAL PURPOSE:
Provides professional social work services to patients and their families. Collaborates with hospital staff and community social services resources relative to the psycho-social problems of patients. Participates in formulating procedures and policies of the Division of Social Service. Participates in educational and research programs, including the orientation and training of new staff and students.
**ESSENTIAL FUNCTIONS**
+ Evaluates the psycho-social status of patients and family to determine specific needs for service. Provides brief or intensive assistance through counseling and direct services to the patient and patient's family. Discusses plans for treatment and disposition with patients, helps patients and family to cope with home and community problems which hinder health care, evaluates and makes recommendations regarding hospital release, home care, financial assistance, use of clinics, custodial institutions and rehabilitation services. Makes home visits when necessary. Refers patient and family to community resources to maintain continuity of service.
+ Provides social service consultation to other members of the hospital staff. Attends rounds and staff meetings to coordinate the efforts of the hospital and clinic staffs for the patient's welfare. Helps develop a rehabilitation plan for individuals through the sharing of knowledge of the patients and mobilization of available resources.
+ Determines whether social work services are needed for patients and, when necessary, seeks further clarification of reasons for referral.
+ Establishes and maintains cooperative working relationships with community agencies which may be helpful to patients.
+ Orients medical students and other hospital staff to the psycho-social aspects of illness and services rendered through the Division of Social Service. Provides information on psycho-social needs and problems of individuals with specific diseases or other medical problems. Teaches casework, group work and community organization to residents in medicine and psychiatry, medical students and nurses, and trainees in psychology and social work.
+ Attends and participates in departmental, hospital, professional and special meetings to keep abreast of social service activities and to share new developments in the Social Service program. Meets with members of community agencies to share information, procedures and techniques. Maintains a thorough knowledge of social work practices and the policies of community agencies through workshops, symposia and social work literature.
+ May supervise lower level support staff and students and evaluates performance. Assists in the orientation and training of new members of the staff through individual and group meetings. Arranges and carries out case conferences on a planned basis.
+ Makes recommendations on procedures and policies of the Division of Social Service. Completes recording on all cases according to established policies. Conducts correspondence with other institutions and community agencies.

**MINIMUM EDUCATION & EXPERIENCE**
+ Master's degree from an accredited school of social work required
+ Or equivalent combination of education and experience
**LICENSES AND CERTIFICATIONS**
+ LMSW - Licensed Medical Social Worker - State Licensure within 1-1/2 years required or
+ If duties will be performed under OMH a minimum of a limited permit (not to exceed 1 year) or NYS Licensure upon hire required
