Job Title: Social Worker
Job Description

The Social Worker provides direct case management services to PACE participants. The primary role of the MSW is to assist participants and their family/caregivers to cope with social and emotional issues and practical arrangements related to maintaining their independence. With support from the Social Work Lead, the MSW serves as an integral member of the Interdisciplinary team (IDT) providing assessments, coordination, treatment planning, information and referral to community resources, and other social work services to meet the complex needs of participants and their families.

The first 90 days are all onsite at the PACE center. After that, it's a hybrid schedule with 3 days a week onsite. The PACE center is an outpatient day center with medical services, activities, case management, and mental health services. The team has a great culture, passionate about supporting underserved populations and the community. This role offers the opportunity to gain supervisory hours and the organization has a strong tenure with most of the team being there for over a decade. The benefits offered are very strong.

Pay and Benefits

The pay range for this position is $93000.00 - $104374.00
