Job Description Good Shepherd Fairview Home is looking for a caring Social Worker to assist in providing the best quality of life for our Long Term Care Residents! Position Summary The Social Worker is responsible for providing social services to resident and their families on the Long Term Care unit including: the ability to achieve their highest level of function; participate in the development pf residents' comprehensive care plans; develop policies and procedures to provide psychosocial services to residents in compliance with federal, state and local regulations and to promote person-centered care for optimal quality of care. Essential Job Requirements Education: Associates in Human Services or Related Field Qualifications: Must be 18 years of age. Ability to read, write and speak the English language Experience: One(1) year providing Human Services preferred Essential Job Duties and Responsibilities - Maintain adequate records and documentation for the Nursing Facility in compliance with state and federal regulations, including assessments, MDS's, and CAA's - Participate in weekly resident care planning conferences. This includes, but is not limited to, preparing necessary care plans and forms, ensuring resident and family participation on a timely basis, transfer and discharge planning, documentation - Participates in Quality Assurance and Performance Improvement (QAPI) process - Assist with the admission process for Nursing Facility and respite care residents including, but not limited to: admission interviews, social histories, assessments, adjustment issues, and documentation - Counsels resident and families in regard to resident related issues - Act as a resident and family contact for resident related grievances, including investigation, interview, and follow up APPLY NOW!
