We are seeking a compassionate and skilled Palliative Medicine Social Worker to join our interdisciplinary team. This role is integral in supporting patients with chronic or life-limiting illnesses and their families by providing psychosocial support, advocacy, and care coordination. The ideal candidate will demonstrate expertise in addressing the complex emotional, social, and practical needs of patients, while promoting dignity and enhancing quality of life.
Key Responsibilities:
Patient and Family Support:
Conduct comprehensive psychosocial assessments to identify patient and family needs. Provide counseling and emotional support to patients and families coping with serious illness. Facilitate advance care planning discussions and assist in documenting goals of care.Care Coordination:
Collaborate with interdisciplinary team members, including physicians, nurses, and chaplains, to develop and implement care plans. Act as a liaison between patients, families, and healthcare providers to ensure continuity of care.Advocacy and Education:
Advocate for patients’ rights and preferences within the healthcare system. Educate patients and families about illness progression, palliative care principles, and available resources. Support families in navigating complex medical and social systems.Program Development:
Participate in quality improvement initiatives to enhance palliative care services. Assist in the development of educational materials and workshops for patients, families, and staff. Contribute to the growth of the palliative care program by identifying and addressing service gaps.Qualifications
Master of Social Work required. Current License to practice as a Master Social Worker, or Current License to practice as a Clinical Social Worker in New York State required, plus specialized certifications as needed.
Skills:
Strong interpersonal and communication skills. Ability to work collaboratively in an interdisciplinary team. Proficient in crisis intervention and problem-solving.Preferred Qualifications
Certification in Hospice and Palliative Social Work (APHSW-C). Experience with culturally competent care and working with diverse populations. Knowledge of local and national resources for palliative and geriatric care.
