Job Summary:
This position conducts initial intake assessments of adult clients and provides information and referral services to clients. This position also provides counseling and support group services to clients and employees.
Duties/Responsibilities:
+ Conducts initial intake assessments. Consults with CABVI staff and contact persons to formulate plans.
+ Schedules assessments after receipt of eye report.
+ Addresses client and family emotional needs in regard to vision loss.
+ Shows warmth and empathy and uses consumer friendly language when conversing with clients.
+ Informs client of appropriate services, accurately assesses client’s readiness for service, and develops plan according to client’s choices.
+ Sets appropriate fee plan.
+ Shows ability to work with diverse population.
+ Achieves an annual direct service percentage of at least 30%.
+ Provides information and referral services coverage as requested to assist team.
+ Responds to intake calls within 1 business day of contact.
+ Provides appropriate, accurate and useful information to caller.
+ Shows warmth and empathy when conversing with clients.
+ Utilizes current knowledge of community resources by appropriately referring callers to various resources.
+ Provides counseling and support group services.
+ Identifies counseling needs and requests assignment of cases.
+ Applies appropriate counseling methods.
+ Facilitates support groups as needed.
+ Maintains accurate and timely information on client service. Prepares other service reports in a timely manner.
+ Prioritizes work for best service and timely completion.
+ Dictates assessments within at least 3 business days of completion of assessment.
+ Provides accurate and complete client information.
+ Submits Day Sheets, mileage reports, progress notes and other reports on a timely basis.
+ Initiate and actively participate in professional activities to help meet program or agency needs.
+ Actively participate in program planning, program development and identification of unmet needs.
+ Thorough preparation for projects and talks is made.
+ Complete projects within a set timeline.
+ Projects meet or exceed anticipated outcomes.
+ Collaborate with other staff as needed.
+ Handle tours and talks when requested.
Qualifications
Education and Experience:
+ Master’s degree in social work.
+ Ohio Social Work License.
Required Skills/Abilities:
+ Good oral and written communication skills.
+ Proficient with use of computer software including Microsoft Word and Outlook.
+ Ability to work and interact with a diverse population.
