Find your purpose as a Social Worker, in an essential role, supporting patients and their families while in the hospital! As an integral part of CentraCare Health – Monticello, the Social Worker will collaborate with the social work team supporting all units at the Monticello hospital including the Geriatric Behavioral Health Unit (GBHU), Hospital Inpatient, and Emergency Room. This role also provides support to the Cancer Center at our outpatient specialty care center connected to the hospital. The social worker provides care to facilitate achievement of expected patient outcomes through a collaborative process of assessment, planning, care coordination, communication and advocacy for options and services to meet individuals' needs. Social workers will ensure compliance with licensing boards, professional standards and accrediting agencies while providing effective, efficient services and ensure appropriate use of resources across the continuum. Social Workers will promote patient/resident and family centered care to achieve CentraCare Health – Monticello's mission working to improve the health of every patient, everyday. Full-time 80 hours every two weeks Work Days are Monday-Friday Shifts are 8:00a.m.-4:30p.m. This position supports the Monticello hospital Social Work Department 8 days per pay period and 2 days per pay period at the Specialty Clinic Cancer Center.Pay and Benefits
Starting pay begins at $58,136.00 per year and increases with experience Salary range: $55,310.74-$82,944.73 per year Salary range is based on a 1.0 FTE, reduced FTE will result in a prorated offer rate $3,000 hiring bonus and $5,000 relocation bonus for eligible candidates! Current Licensed Social Worker (LSW) in Minnesota required. Bachelor's degree in social work. At least two years of experience as a Social Worker, preferably in a healthcare setting. Knowledge of Joint Commission, MN Department of Health standards and various other regulatory standards. Ability to read and communicate effectively in English. Strong written and verbal skills. Strong interpersonal human relations skills.Hiring bonus requirements: To be eligible for the hiring bonus you must be an external candidate who is hired into an eligible full time or part time benefited position.
