Job Title: Social Worker
Job Description

Coordinates patient care services and resources to assure optimal clinical quality and financial outcomes. Works collaboratively with the Case Management Coordinator and other health care team members. Uses screening mechanisms to identify patients who are at risk for lengthy inpatient stays, complex clinical issues, high resource use, or repeat admissions due to chronic diseases. Intervenes within 24 hours of patient admissions and documents according to established guidelines.

ResponsibilitiesCoordinate patient care services to ensure optimal clinical and financial outcomes.Collaborate with Case Management Coordinator and other healthcare team members.Use screening mechanisms to identify patients at risk for lengthy inpatient stays, complex clinical issues, high resource use, or repeat admissions due to chronic diseases.Intervene within 24 hours of patient admissions.Document patient information according to established guidelines.Essential SkillsBachelor’s Degree in Behavioral Health, Social Work, or related field.Maryland Social Work licensure required or LSWA.Experience as a social worker in a hospital environment preferred.Additional Skills & QualificationsCurrent Maryland LCSW preferred.Bilingual.Experience working with providers and insurance.Managed care and case management.Health care and discharge planning.Work Environment

Hospital setting. Office. Monday - Friday, 8:30am - 5pm.

Pay and Benefits

The pay range for this position is $35.00 - $48.00/hr.
