Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations._ **Responsibilities:** GENERAL PURPOSE: The social worker assigned to the Strong Ties Clinic is responsible for providing immediate psychosocial assessment and social work treatment to individuals who are severely and persistently mentally ill. This is provided through participation on an interdisciplinary team designed to provide comprehensive community based psychiatric care. The social worker functions as a crisis intervention worker for patients enrolled at Strong Ties, providing direct clinical service including individual, family and/or group therapy, crisis intervention, case management, and community outreach. **ESSENTIAL FUNCTIONS** + Provides direct clinical services to assigned patients, which may involve individual, family, and/or group therapy, crisis intervention, and diagnostic evaluations. Works collaboratively with the Crisis Team Lead to support clinic access. Provides coverage and accessibility for intakes, vacations, and sick calls as needed + Participates as a member of an interdisciplinary team involving consultation, treatment planning, and periodic care review. + Collaborates with family, social supports, community agencies (past and present) to ensure appropriate and effective care. Coordinates referrals and facilitates linkage to other appropriate agencies/supports. + Documents interventions, both direct and indirect, in the patient’s medical record in compliance with the Division, Department, and Hospital policy. + Attends Division, Department, and Clinic level staff meetings. Ensures that Division, Department, and Clinic policies and procedures are implemented with regard to professional practice and patient care. Ensures timely completion of statistical reports/forms, time sheets, and other Hospital or Division mandatory in-service trainings and Health Assessments. Subscribes to maintaining a culturally competent environment by treating patient, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences. Participates in available cultural competency events and training appropriate to job duties. + Develops and leads groups for patients (and possibly family members or their support network) which focuses on issues and needs of the severely and persistently mentally ill population + Provides back-up coverage for patient care, as appropriate and requested. Other duties as assigned **QUALIFICATIONS** + Master’s Degree in Social Work from an accredited social work graduate program required. + Clinical experience working with individuals with chronic mental illness and/or community mental health preferred. + Experience in the management of psychiatric crisis preferred. + Knowledge of mental illness preferred. + Ability to engage in person centered treatment preferred. + Knowledge/skills in various therapeutic interventions including CBT, DBT, motivational interviewing preferred. + Knowledge of risk evaluation and mitigation preferred. **Required:** + Master's degree from an accredited school of social work.
