SUMMARY Assists physician and health care team members in understanding the significant social and emotional factors related to the health problems; participates in the development of the plan of treatment; prepares clinical and progress notes; works with the family; utilizes appropriate community resources; leads patient groups; and participates in discharge planning. The SW will be assigned to the Behavioral Health Unit and work with both the geriatric and adult populations. Qualifications include: ? Bachelor’s Degree in Social Work required, Master’s preferred. ? License in Social Work required ? Knowledge regarding community resources required. ? One year of hospital/health care experience preferred. ? Excellent communication skills and ability to work in a collaborative team approach required.
