As a Social Worker at CHI St. Vincent Little Rock, you will be a part of a dynamic team that works together to provide comprehensive care coordination for acute care patients to ensure a safe, timely, and appropriate discharge.

Responsibilities
Work with Registered Nurse Manager to provide timely and efficient identification of patients who need discharge planning. Provide assessments of psychosocial impact of illness.Provide direct services including discharge planning for patients needing community resources prior to discharge from the acute care setting. Experience in Hospital/Acute Care experience preferred.
Qualifications
Education: Bachelor of Social Work (BSW) degree required.A current unencumbered Social Worker license to practice in the State of Arkansas is requiredBLS within 30 days Must be able to communicate effectively, both written and oral.Must possess organizational skills, able to prioritize work.Must possess demonstrated training, knowledge, and skills necessary/appropriate to the adult, and/or geriatric age groups
