About the Job

The Emergency Department Social Worker is responsible for providing comprehensive behavioral health services to patients and their families in the Emergency Department. This role focuses on the integration of SBIRT (Screening, Brief Intervention, and Referral to Treatment) with mental health care, addressing both substance use and co-occurring mental health disorders.

Requirements:

Bachelor (BSW) degree in Social Work from an accredited program required.Master (MSW) degree in Social Work from an accredited program preferred.Minimum of 2 years of experience in a healthcare setting or a behavioral health environment, with a focus on substance use and mental health disorders.Experience with SBIRT or similar programs is highly desirable preferred.Bilingual a plus

Responsibilities include:

Screening and Assessment: Conduct initial screenings using validated tools to identify patients at risk for substance use and mental health disorders. Perform comprehensive biopsychosocial assessments to evaluate the interaction between substance use and mental health issues. Brief Intervention: Deliver evidence-based brief interventions, such as motivational interviewing (MI) and cognitive-behavioral strategies, to address substance use and mental health concerns. Engage patients in discussions about their substance use and mental health, providing support and motivation for behavior change. Referral to Treatment: Identify and refer patients to appropriate treatment resources, including inpatient, outpatient, and community-based services that address both substance use and mental health disorders. Coordinate with treatment providers to ensure seamless transitions and continuity of care.Care Coordination: Collaborate with multidisciplinary teams, including physicians, nurses, and mental health professionals, to develop and implement comprehensive care plans. Follow up with patients post-discharge to support engagement in treatment and monitor progress. Initiate referral to patients assigned MCO to request Care Coordinator.Crisis Intervention: Provide immediate support and stabilization for patients experiencing crises related to substance use or mental health issues. Implement de-escalation strategies and emergency response protocols as needed.Patient and Family Education: Educate patients and their families about the relationship between substance use and mental health, treatment options, and the recovery process. Develop and distribute educational materials tailored to diverse populations.Advocacy: Advocate for patients’ needs within the healthcare system and the community to ensure they receive integrated, compassionate care. Assist patients in navigating healthcare and social service systems to access necessary resources.Ethical and Cultural Competence: Adhere to ethical standards and demonstrate cultural competence in all interactions with patients and their families. Participate in ongoing education to enhance understanding of cultural, ethnic, and socio-economic factors influencing substance use and mental health.Maintain positive working relationships with medical staff and other hospital employees throughout the continuum to ensure appropriate patient care.Demonstrate compliance in documentation, related to State, Medicare, and TJC regulatory requirements.Organizes and completes work in an accurate and timely manner. Responds to requests appropriately and takes initiative. Consistently meets deadlines.Adheres to federal, state and local regulations and accreditation requirements impacting social work scope of services. Adheres to department structure and staffing, policies and procedures to comply with the CMS Conditions of Participation and MountainView Regional Medical Center.Computer literate in computer applications, specifically Word, Excel, PowerPoint, Adobe & hospital wide and/or department specific computer software and technology.Performs other job duties relevant to the position as assigned.
