Works in collaboration with multidisciplinary teams, coordinates patient discharge planning, complies with required standards for medical/legal responsibilities, and serves as a resource to other allied and administrative staff. **Requisition ID:** 39854BR **Optional Attachments:** Transcript **Travel Required:** Up to 25% **Major/Essential Functions:** + Investigate, assess and plan interventions to help patients cope with social, emotional, economic and environmental problems. + Interview patients, assess priorities and document case activity. + Consult with team managers, statutory and voluntary agencies and patient's relatives to ensure compliance with patient's treatment plan. + Assist clients/families with obtaining community assistance by referral to proper resources. + Develop and maintain working relationships with community health, welfare and social agencies and seek creative means to assist the client with his/her needs. + Support management of concurrent review, discharge planning processes, social services, home care and other post discharge needs. + Adhere to all policies, procedures and practices (Regents Rules, TTUS, HSECEP OPs, Community Living Assistance Support Services Program/HHSC, etc.). + Personally demonstrate, display and act in accordance with TTUHSC EP's Values (Service, Respect, Accountability, Integrity, Advancement, and Teamwork).
