By the order of the Physician or Case Manager, assesses the discharge, social service needs of the elderly, disabled, financial needy patients, including counseling those with risk for financial/emotional/physical abuse/ exploitation. Makes referrals to approach social service agencies and arrange for placement as directed. Consults with MSW SW as needed for clinically complex patients or difficult family dynamics. Qualifications: In addition to bringing humankindness to the workplace each day, qualified candidates will need the following: Bachelors Degree in Social Work, Professional Counseling, Marriage, Family and Child Counseling 1-3 years of experience in Social Work with emphasis on discharge planning, referral to community resources and/or case management. If licensure required in the state of employement. St. Elizabeth Hospital, located in Fort Morgan, Colorado, is a Level 3 Trauma Center and an American College of Cardiology Accredited Chest Pain Center serving northeastern Colorado. Providing care through-out the region, St. Elizabeth Hospital provides cardiology, maternal, emergency, surgical, pediatrics, and acute care inpatient services in addition to the multitude of subspecialists in our specialty clinic. At 50 beds, St. Elizabeth is the region’s referral center serving a wide span of northeastern Colorado. Since 1952, St. Elizabeth has been serving the community in Morgan County which is one of the most diverse counties in Colorado speaking more than 28 languages.
