**Social Worker - BSW (Remote in Indiana - FT - Days)

We are looking for a BSW to join our Virtual Integrated Behavioral Health Team. The BSW team works remotely to outreach patients and manage referrals to our brief therapy, collaborative care program.  The team is engaged and supports each other through the virtual environment. The BSW is a core member of the virtual integrated behavioral health care team, along with the patient’s medical provider, behavioral health counselor, psychiatric consultant, and patient. The BSW completes behavioral health intakes and enrolls patients in collaborative care management.  The BSW effectively engages patients during intake while using evidence-based tools to screen for safety and acuity. In addition to scheduling appointments, the BSW is responsible for maintaining data entry to monitor patient referrals. 

  DUTIES AND RESPONSIBILITIES 

1. Complete virtual intakes for patients referred by primary care providers for brief, virtual therapy.   

2. Manage depression, anxiety, and suicide referral registries, including order and data entry to ensure patients are receiving behavioral health care.   

3. Screen patients for acuity of symptoms for triaging to appropriate level of care. 

4. Collaboratively work with Behavioral Health Specialists in managing referrals, answering phones, and scheduling patient sessions.   

5. Utilize evidence-based screeners to screen suicide risk and collaborate with team members as needed to ensure patient safety and appropriate level of care for patients. 

7. Facilitate referrals for clinically indicated services outside of the organization (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care, substance abuse treatment). 

 

****Remote working out of the state of Indiana 

• 0-3 years of experience required. Bachelor's Degree in Social Work required.
• Permanent LSW licensure preferred.
• Requires strong clinical assessment, planning and goal development skills.
• Requires knowledge of state and county adult and child protective legal mandates.
• Requires knowledge of the principles of growth and development over the life span.
• Requires strong counseling skills to help patients and families understand, accept and follow medical recommendations.
• Requires proficiency in Microsoft Excel and Word.
