Discover a rewarding career at AJH, where you can make a meaningful impact within a close-knit community while advancing your professional growth within a thriving healthcare system._ **_Are you ready to become a part of our dynamic Hematology and Oncology Team?_** Responsibilities include but are not limited to: + Identifies new admissions to Oncology and reviews for clinical appropriateness and treatment plan. + Develops an individual plan of care based on the needs of the patient and develops a realistic discharge plan in collaboration with the patient/family/health care team. + Implements a plan of care based on the needs of the patient, providing ongoing support and resources to patients and families. + Effectively communicates with the interdisciplinary health care team discharge needs and obstacles affecting treatment and discharge disposition. Makes referrals to appropriate agencies based on patient need and sets up family/resource meetings. + Expresses alternative points of view in a non- threatening, respectful and professional way. Acknowledges when it is appropriate to compromise and when it is important to take a stand. + Sensitive to the needs, feelings and capabilities of others. Approaches others in a non- threatening and pleasant manner, shows tact and diplomacy. + Provides support and consultation to other members of the health care team around psychological, social and management issues of patients. **Scheduled Hours:** 16 hr/wk **Work Shift:** Day - 2 x 6hr days, 1 x 4hr day, **Education:** High School Diploma or equivalent required. **Required Experience:** Clinical Social Worker experience, minimum of one year in an Oncology setting preferred or related experience.
