This position will case manage our short-term patients to assist in assuring they get the maximum benefit in their authorized stay.  This includes extensive discharge planning and a successful and smooth return to the community.

Candidates must have at least two years' experience with case management in a medical setting.  Skilled nursing facility experience is highly preferred. 

Candidates may be trained in nursing or social work.

Wage range is $25 to $35 DOE

Mon-Fri - FT- Day shift - occasional weekend
