The Social Worker / Case Manager will identify patients of WRMC appropriate for Case Management according to criteria determined by the Case Management Team. Monitor and coordinate continuum of care for patients. Work collaboratively with the Case Management Team, WRMC staff, physicians, community members and other service providers to utilize and develop services that most appropriately meet patients' needs, while striving to sustain and/or improve the quality of life. Perform utilization review, and participate in medical management program (pathways, protocols and guidelines) as appropriate.

Qualified candidates will have\:
Bachelor’s Degree in Social Work, required. Master's preferred.
Three years’ experience in healthcare.
Competent in the utilization review processes.
Licensed in the state of Wisconsin as a Certified Social Worker, required.
