Social Worker-Children's Residential Program

Essential Job Functions: Provide outreach to families though phone contact, in person, written correspondence, conduct parent workshops, lead parent meetings and direct interaction during family crisis. Interpret governmental regulations relating to the individual’s rights and ensure that the individual’s rights are upheld in all programmatic as well as non-programmatic areas. Flexibility in work hours is necessary.

Responsibilities:

Assist with the intake of students and associated paperwork. Act as liaison person with outside resources, (i.e. other DDI programs, DDI Transportation, other agencies, etc.) Maintain documentation in a professional, efficient, accountable and confidential manner, according to procedures/requirements outlined by the program/supervisor/regulations. Coordinate and participate in activities related to planning for program transitions. (Transition across to Adult) Provide information, referral, advocacy and support to staff members and their families, especially in the area of guardianship. Participate in quarterly, semi annuals and annual inter disciplinary team meetings Participate in Agency-wide activities (committees, training, supervision of student interns from area colleges and universities) as needed for the purpose of supporting and furthering the Agency mission to provide service to the developmentally disabled and their families.

Essential Physical Demands:

Must be able to modify the area to secure the safety of the children i.e. move/push tables and other heavy objects up to 20 lbs. Must be able to lift/move 20 lbs. (mats) Must be able to run after student up to 500 feet Must be able to run to a classroom in need up to 500 feet Must be able to kneel, twist and bend Must be able to get up and down from floor
